Chandler Center for the Arts (CCA) has permission to use any and all photographs taken of my child and to include his/her name in materials that promote CCA services or CCA programs, or to publicize any event without payment or notification. I understand that all prints and digital files become the sole property of the CCA and may be used by the CCA without payment or notification. I understand that my child will be participating in activities that could include radio, TV, or internet coverage. By attending this camp, you agree to allow their likenesses and images either in audio, photographic, or video format recorded onsite to be used by CCA for educational, promotional, marketing, advertising, or commercial purposes.

DISCLAIMER: BY SIGNING THIS LIABILITY RELEASE, YOU ARE EXPRESSLY RELEASING AND WAIVING ANY RIGHTS YOU MAY HAVE TO TAKE LEGAL ACTION AGAINST THE CHANDLER CENTER FOR THE ARTS, EMPLOYEES OR AGENTS FROM ANY LIABILITIES, CLAIMS, INJURIES, OR DAMAGES OF ANY KIND OR NATURE, WHETHER FORESEEABLE OR UNFORESEEABLE, WHICH IN ANY WAY RELATE TO OR INVOLVE YOU OR YOUR FAMILY MEMBER’S PARTICIPATION AT CAMP.  PARENTS ARE SOLELY RESPONSIBLE FOR ENSURING PROPER PICK-UP OF STUDENTS FOLLOWING CAMP.

Individuals permitted to pick up child

At the bottom of this form, please list any person who might be picking up your child to avoid any embarrassment or inconvenience at the check-out table.  Please include parents on this list. Parents are permitted to allow their child (14 years & up) to check themselves in and out of camp by noting the child’s information is in the drop off/pick up section below.

Please Note: Check-in will begin no earlier than 8:45am. For pick up, please arrive by 3:30pm. If you are late picking up your camper, they will be checked in to After Camp Care and you will be charged $15 per camper. After Camp Care is from 3:30pm-4:30pm.
